Statement of Understanding of Part-time Employment Status

I hereby acknowledge that I understand that the position I am applying for is as a part time employee with Abundant Life HCS. As a part time employee I recognize I will not have any of the following benefits, to include but not limited to, insurance, paid vacation or paid sick leave provided by Abundant Life HCS. I further acknowledge that I realize part time means no more than 29 hours per week. I am aware that I will work no less than 2 hours per week and that I may not be called to work more than that during any pay period. I know my hours may be flexible and that I may be called on short notice to fill vacancies as needed. Finally I understand that if I am called and do not answer (two or more times), do not show or call in for my scheduled shift or refuse to come in without a valid reason, i.e. sick, emergency, etc, this will serve as my notice that I have terminated my relationship with Abundant Life HCS.
