Driver License Division License Eligibility I do hereby authorize Abundant Life HCS Company to request the status of my driver’s license for purposes in determining eligibility to operate the company’s vehicle. I do understand that if my license status deems me ineligible to drive (license suspension), management will be notified and I will not operate any vehicle owned to Abundant Life HCS for any reason. I also acknowledge that I will provide Abundant Life HCS Company with any documentation or proof that shows that my license status is no longer ineligible and in good standing,
